To help us tailor certain areas of the Site to your needs and interests, we will ask you to provide some personal information when you complete the registration form. In addition, you will be asked to supply personal information, including personal health information, when you enroll in personalized tools, trackers and services available on the Site. When you are requested to register or enroll in certain areas of the Site but choose not to provide this information, you may not have access to the tools, trackers and services in these areas and, if you do, such tools, trackers and services may not be optimized for your use. We recognize the importance of protecting your personal information, including your personal health information, and keep that information stored in our secure database. Please refer to the "Security” section of this Policy for additional information.
Demographic Information
We may also collect demographic information from you, such as age, gender, and areas of interest, as well as information regarding the "IP address" that is used to identify your computer on the Internet and information obtained by tracking your usage of our Site. We use this information to improve our Site, by measuring interest, traffic, and usage patterns.

If at any time, you wish to permanently remove any personal information from our system, please contact us directly at the email address provided at the end of this Policy.
We Will Use Your Personal Information to Contact You
We will use your personal information to contact you, if necessary, or, if you choose to receive email from us, to send you newsletters and to inform you about special offers or events that might interest you. In the event the email address you provide to us is your work email account or is otherwise provided by your employer, you expressly acknowledge that your employer may have access to any emails sent to that address in accordance with your employer's policy on monitoring employee email. In addition, Revive reserves the right to de-identify and aggregate data, and to use such aggregated data in accordance with all applicable laws. If your membership is sponsored by your employer or another third party, we may share information with that party in de-identified, aggregate form only. For example, we may report to your employer the number of its employees using the Site or the frequency of their collective use. This information will not identify you personally. We may learn of third parties who provide products or services that we believe may be of interest to you. If so, and only if you have agreed to receive email from us, we may notify you on such third parties' behalf. You always have the option to change your mind about receiving email from us. Simply follow the unsubscribe directions in the footer of our newsletters and promotional emails.
In general, we do not share your personal information with your employer. However, if your employer has contracted with us to provide services, we may provide personal information to certain individuals designated by your employer to have access to such information in order to perform certain administrative functions on behalf of your employer. Your employer may elect to offer financial incentives to you to encourage your participation in the services offered through the Site. In such event, your employer may be informed if you qualify for the financial incentive based upon your participation, but we will not provide your employer any of your personal information, except as set forth above.
Security
We have security measures in place to protect against the loss, misuse, and alteration of the information you provide us. We use SSL (Secure Socket Layer) to encrypt and password protect all information transmitted to or from the secure areas of the Site. We have implemented and maintain administrative, physical and technological safeguards that reasonably and appropriately protect the confidentiality, integrity and availability of all personal information, including protected health information, submitted by you through the Site.
Your Responsibility
Although we will protect your privacy on the Site as described in this Policy, you should keep in mind that if you give others access to your computer, your password or disclose personal information online, it is possible that despite our efforts information may be accessible by others. You can help guard against this by never disclosing your password, giving anyone else access to your personal computer, and by ensuring that information you disclose online is transmitted securely. You should also refer to and comply with the "Password and Security” section of the Agreement.

The Information Available Through the Site Is Not a Substitute for Professional Advice
The services and information that we provide on or through the Site are not substitutes for and do not replace the services of trained professionals in their fields of expertise, including, without limitation, with respect to financial, health and medical or legal matters. We recommend consulting with a healthcare professional on a regular basis with respect to any matter relating to physical or mental health, especially concerning symptoms that may require diagnosis or medical attention. Neither Revive nor any of its affiliates, licensors, vendors or suppliers make any representations or warranties concerning any treatment, action or application of medication or preparation by any person following the information provided on or through the Site. Please refer to the "Disclaimer” provision below for additional information.
Content, Products and Services
Revive reserves the right to change or discontinue any content, products, and services made available through the Site at any time and without any notice to you. Certain content, products, services and tools discussed in this Agreement and the Privacy Policy may not be available to you because of the way your particular Site is customized.
Tools and Trackers
Certain tools and trackers may be available to you through the Site. The accuracy and efficacy of these tools and trackers are dependent on the information you enter being true and correct. Please update your profile if any of your information changes to ensure that the tools and trackers work properly.
We strongly recommend that you consult with an appropriate healthcare professional about any safety or medical questions you might have before starting or terminating any treatment or program. In particular, certain individuals should not begin any exercise or diet program without first consulting a health care professional according to the National Institutes of Health. If any of the following apply to you, we require that you seek medical supervision before beginning a new exercise program: (i) a health care professional has ever told you that you have heart problems; (ii) a health care professional has ever told you that you have high blood pressure; (iii) a health care professional has ever told you that you have a bone or joint problem, such as arthritis, that has been or could be aggravated by exercise; (iv) you have a serious chronic disease such as congestive heart failure or diabetes, or you are at high risk for any chronic disease - for example, you smoke or have a family history of heart disease or diabetes, or you exceed your optimal weight by more than 20%; (v) you have had a heart attack; (vi) you frequently suffer from chest pains; (vii) you often feel faint or have dizzy spells; (viii) you are over age 65; (ix) you are taking prescription medications, such as those for high blood pressure; or (x) you have a good medical reason, not mentioned here, why you should not exercise. Finally, if you are a man over 40, or a woman over 50, and none of the above apply to you, you should still see a health care professional if you intend to start engaging in vigorous exercise.
Tools such as the Meal Planner and the Diet Manager, which you may have access to through the Site, provide information and recommended meal plans based on your personal profile. These tools are not intended to replace the advice of a health care professional and are not recommended for individuals with certain chronic health conditions. This includes individuals who have had bariatric surgery; those recently diagnosed with diabetes or those who take insulin for diabetes; and those with chronic digestive diseases, eating disorders, renal disease and other conditions.
Consult with an appropriate healthcare professional before using these tools if you are over the age of 80, pregnant, lactating, need to lose more than 30 pounds, or are on a special diet prescribed by your health care professional. Do not use these tools to lose greater than 2 pounds per week. You may download or print a copy of any information provided to you as part of the Meal Planner or the Diet Manager. This information is provided for your personal use only. Any other use in downloading or printing a copy of any information obtained through the Meal Planner or the Diet Manager is expressly prohibited.
We may deny you access to the Meal Planner and/or the Diet Manager at any time for any reason at our sole discretion.
